In Ayurveda, the management of Parikartika involves a comprehensive approach that includes dietary and lifestyle modifications, topical applications, and herbal remedies and surgical intervention is required in some cases.
Here are some key aspects of Ayurvedic management for Parikartika(Fissure in ano):
- Dietary and Lifestyle Modifications: Ayurveda emphasizes the importance of maintaining healthy digestion and promoting regular bowel movements. Recommendations may include increasing fiber intake through fruits, vegetables, and whole grains, staying hydrated, avoiding excessive straining during bowel movements, and maintaining proper hygiene of the anal area.
- Sitz Bath: A sitz bath is a common Ayurvedic remedy for Parikartika. It involves sitting in warm water or a herbal decoction that helps soothe the anal area, reduce inflammation, and promote healing.
- Herbal Remedies: Ayurvedic practitioners may prescribe various herbal formulations or ointments to be applied topically to the anal area. These formulations may contain shatdhaut gritam ointment, jatyadi malam, jatyadi oil, jatyadi gritam etc. which are believed to have anti-inflammatory, analgesic, and wound-healing properties.
- Stool Softeners: In cases where constipation is a contributing factor, Ayurvedic practitioners may recommend the use of natural stool softeners or mild laxatives to promote regular and easy bowel movements. This helps reduce strain during defecation and aids in the healing of the anal fissure.

- Surgery of Fissure in ano: In certain cases, surgical intervention may be necessary to treat chronic and recurrent fissure cases which is associated with sentinel tags or with anal stricture or with anal stenosis. The most common surgical procedure is Partical Sphincterotomy with Lord’s Dilatation in such cases.
Partial Sphincterotomy: In this procedure, a small portion of the internal anal sphincter muscle is surgically divided or partially cut. The purpose of this is to reduce the sphincter tone, which helps alleviate the spasm and tension that contribute to the persistence of the fissure. By reducing the sphincter pressure, it promotes healing and reduces pain during bowel movements.
Lord’s Dilatation: Lord’s dilatation is a technique used to stretch the anal canal. After the partial sphincterotomy, an anal dilator or speculum is gently inserted into the anus and gradually widened, thereby stretching the anal canal. This helps to promote healing, improve blood flow to the fissure, and reduce anal sphincter spasm.
The combination of partial sphincterotomy and Lord’s dilatation aims to relieve the spasm, reduce pressure on the fissure, and promote healing of the anal fissure. The procedure is typically performed under local anesthesia or spinal anesthesia, and the recovery period can vary from a few days to a few weeks. During the recovery period, patients are usually advised to follow proper anal hygiene, maintain regular bowel movements, and make dietary and lifestyle modifications to prevent constipation.
